below and complete it before your initial visit. For returning patients, use
the form to update your address, telephone number, new insurance information or any other relevant data we may have on you in our facility. If you have been seen in the office, chances are that
you have been given discharge instructions for such things as wound care, sprains,
back pain etc, now you can remain paperless by retrieving same information on our
